To report a claim online, please select the type of claim that you are submitting from the options below.

Report a claim over the phone, call 888.762.3083

Report a claim via fax.  Please fax the information below to 612.766.3860.

• Correct ACORD for line of coverage
• Contact Person – Include phone and fax number
• Update your ACORD to match the policy (vehicle numbers, locations, etc.)
• Show what you believe to be the correct deductible and coverage involved.
• If there is blanketed coverage, indicate on the loss report.
• List Rental Reimbursement if you believe there is coverage.
• All supporting documents received from the insured/claimant.  If there is a lawsuit, include the following information on your ACORD:
• Method of service; via mail, in person, etc.
• If service was in person, who was served?
• Date of service
