Human Services Claim Reporting

In today’s world it is not a matter of if, but when, you will be faced with an insurance claim. When you do, Riverport will be there for you. Claims are handled in a timely and professional manner. To the extent possible, we attempt to work with our insureds to achieve cost-effective management and resolution of claims.

Insureds: Please report claims directly to your insurance agent for the fastest response.

Agents: Please fax the required claims information, as outlined below, to (612) 766-3860 or call (800) 449-7707. For agents in Arizona, California, Hawaii, New Mexico, Nevada, Oregon or Washington, please fax the required claims information to (415) 898-9656 or call (800) 361-1075.

Please provide the following claims information:
  1. Correct ACORD for line of coverage
  2. Contact Person – Include phone and fax number
  3. Update your ACORD to match the policy (vehicle numbers, locations, etc.)
  4. Show what you believe to be the correct deductible and coverage involved.
  5. If there is blanketed coverage, indicate on the loss report.
  6. List Rental Reimbursement if you believe there is coverage.
  7. All supporting documents received from the insured/claimant.  If there is a lawsuit, include the following information on your ACORD:
    1. Method of service; via mail, in person, etc.
    2. If service was in person, who was served?
    3. Date of service
